If you are a full-time associate, you are eligible for health and welfare benefits on your 31st day of employment. You may also enroll your eligible dependents, including a same-sex domestic partner, in the medical, dental, vision and optional life insurance benefits.

MEDICAL PROGRAM

Limited Brands offers medical and pharmacy coverage through UnitedHealthcare (UHC). Under UHC we offer a choice of managed care medical plans that vary based on price, risk, coverage and predictability. You choose the plan that fits your lifestyle and financial needs.

Our medical programs are available based on your home zip code. The majority of associates are in a zip code covered by the UHC "Choice" plans. If you don't live in an area where the UHC "Choice" plans are available, or if there are not enough providers for an effective network, we offer additional plans to cover your needs.

In order to receive the highest level of benefits under the managed care plans (referred to as network level benefits), the covered person must receive care from a network provider.

You can also use non-network providers, however most covered expenses are paid at a lower percentage after higher annual deductibles.

Prescription Drug Program
Regardless of which medical plan you choose, you'll receive pharmacy benefits. Pharmacy benefits are payable for prescription drugs when a prescription is filed or refilled at a retail network pharmacy or when using the Home Delivery/ On-Line Pharmacy service. This means once the deductible is met, you pay only the prescription copay (or the actual cost if it is less than the co-pay).

DENTAL PROGRAM

Limited Brands offers dental coverage through MetLife. We offer one plan with two ways to use it: in-network coverage or out-of-network coverage. You'll receive higher coverage, as well as lower deductibles if you select an in-network dentist. This means your out-of-pocket costs will be less.

BASIC TERM LIFE INSURANCE

Your Group Term Life Insurance benefit is equal to one times your annual base salary. This is a company paid benefit and is automatically provided to you whether or not you enroll in the medical plan.

OPTIONAL & DEPENDENT LIFE INSURANCE

If you want to increase your total life insurance coverage beyond the Basic Term Life Insurance that Limited Brands provides, you may choose and pay for Optional Life Insurance coverage for you, your spouse/same sex domestic partner and/or children. Optional Life Insurance is offered through MetLife.

DISABILITY

Short Term Disability
Benefits begin on the 8th calendar day of a continuous eligible illness and can continue through the 30th calendar day of illness. You receive 100% of your regular base pay.

Long Term Disability
If you are disabled from work for more than 30 days, you are eligible to receive 60% of your base salary up to a maximum benefit of $11,000 per month. This is a company paid benefit and is automatically provided to you whether or not you are enrolled in the medical plan.

Limited Brands offers a vision plan option or a company paid vision discount for you and your dependents.
